0~18세 소아청소년 권장 기본 예방접종 스케줄 Recommended immunization schedule for 0~18 year old (2021 in the USA) (1) 참조
  • 수막구균(Meningococcus/수막염균) 감염으로 수막구균 수막염(수막구균 뇌막염)이 생길 수 있고,
  • 수막구균이 전신의 혈류를 따라 전신에 감염되면 수막구균 혈증, 또 수막구균 패혈증이 생긴다.
  • 수막과 전신에 동시 감염되면 수막구균 수막염(뇌막염)과 수막구균 혈증이 동시 생길 수 있다.
  • 수막구균 혈청균종에 따라, A군 수막구균, B군 수막구균, C군 수막구균, Y군 수막구균과 W-135군 수막구균 등으로 수막구균을 혈청균종을 분류한다.

  • 군대, 대학교 또는 탁아소 등에서 집단생활을 하는 소아청소년들이나 성인들은 수막구균 감염병에 더 잘 거릴 수 있다.
  • 모든 11세 이하 아이들은 수막구균 예방 백신을 기본적으로 받지 않고 있으나 미국에서는 수막구균 감염으로 인한 감염병을 예방하기 위해 건강한 11세 이상 소아청소년들은 기본적으로 접종받으라고 2005년부터 권장하고 있다.

미 FDA Neisseria meningitidis 혈청형 C, Y와 Haemophilus influenzae type b (Hib) 로 생기는 패혈증과 뇌수막염을 예방 할 수 있는 종합 백신이다.

수막구균 감염으로 인한 감염병 백신 예방접종 방법
  • 소아청소년 건강검진을 받고 열이 있나 체온을 재고,
  • 발열성 감염병이 없고
  • MPSV4 백신이나 MCV4 백신에 알레르기가 없고
  • 그 백신으로 예방접종을 받는데 금기 조건이 없으면
  • 1차 접종 받는다.
  • 3세나 3세 이전에 MPSV4 백신으로 1차 예방접종을 받았고 아직 건강상 고위험군에 속하면 MCV4 백신으로 예방접종을 받아도 된다.표 3-6.수막구균 감염병 예방접종 백신 중 MCV4 백신과 MPSV 백신의 차이

수막구균 감염병 백신 예방접종의 효과
  • ENOMUNE-A/C/Y/W-135 예방접종 백신으로 1차 접종을 받은 후 10~14일 경부터 수막구균 면역체가 나타난다고 한다.

특히 다음과 같은 경우 수막구균 백신 예방접종을 받아야 한다.
  • 미국에서는 수막구균 감염으로 생긴 감염병을 예방하기 위해 건강한 11세 이상 사춘기 아이들에게 통상적으로 예방접종을 받으라고 2005년부터 권장하고 있다.
  • 비장이 없거나 비장 기능이 비정상적인 무비장을 가진 2세 이상 소아청소년들
  • 말기 보체 결핍증이 있는 2세 이상 소아청소년들
  • 수막구균성 감염병이 많이 유행하는 지역이나 나라로 여행가는 사람들
  • 살고 있는 지방에 수막구균 감염병이 발생됐을 때
  • 신병들, 대학생들, 특히 기숙사 생활하는 대학생들
  • 병원 임상 검사실에서 일하는 사람들
  • 그 외
수막구균 감염병 백신 예방접종의 연령
  • MENOMUNE-A/C/Y/W-135 예방접종 백신으로 2세 이하 영유아들에게는 예방접종을 해주지 않는다.
  • 모든 건강한 11세 이하 소아청소년들은 수막구균 백신으로 통상적 예방접종을 받으라고 권장하지 않는다.
  • 그렇지만 11~12세 이상 사춘기 아이들이나 성인들은 누구든지 접종을 받을 수 있다.

  • 수막구균 예방접종 백신에 알레르기가 있거나 발열성 감염병을 앓고 있을 때는 예방접종을 받을 수 없다.
수막구균 감염병 백신 예방접종의 부작용 
  • 예방접종 주사 백신으로 접종을 받은 후 주사 맞은 부위에 통증, 발적, 부종 등이 생길 수 있다.
  • 드물게 알레르기 반응이 생길 수 있다.

수막구균 감염병 백신 예방접종에 관한 추가 정보
  • 침입성 수막구균 감염병이 연간 2200~3000명의 미국 사람들에게 발생되었고
  • 십만 명의 영아들 중 0.9~1.5명,
  • 2~4세 유아들 중 1.8명,
  • 5~17세 유아,
  • 학령기 아이들, 사춘기 아이들 중 0.7명, 18~34세 청장년들 연령층 중 0.7명에게 발생됐다.
  • 수막구균의 종류는 혈청 균종에 따라 13종으로 세분한다.
  • 대부분의 침입성 수막구균 감염은 A, B, C, Y, W-135 혈청균종에 속하는 수막구균의 감염으로 생긴다.
  • A, C, Y, W-135 혈청균종에 속하는 수막구균 감염으로 생긴 감염병을 예방할 수 있는 예방백신이 있다.
  • 미국에서는 아직 수막구균 감염으로 생긴 감염병을 예방하기 위해 11세 이하 건강한 소아청소년들에게 예방접종을 하라고 통상적으로 권장하지 않는다. 그러나 새접종 백신이 방금 나왔다.

  • 엄마아빠들이 소아청소년 자녀들이 뇌수막균 감염의 예방접종을 받기를 원하거나, 이 병이 유행하는 나라에 여행가는 사람들은 예방접종을 받으라고 권장한다.
  • 침입성 수막구균 감염으로 심하게 앓다가 회복되거나 사망한 자녀들의 엄마아빠들의 대부분은 “이 병을 예방할 수 있는 예방접종 백신이 있었다는 사실을 알지 못하고 있었다.” 는 연구도 있다.
  • 이런 종류의 예방접종 백신이 있고 침입성 수막구균 감염병을 예방해 줄 수 있다는 사실을 모든 엄마아빠들이 알고 있으면 좋겠다고 생각한다.
   뇌수막구균 감염병 예방접종 10/2015

  • 미 소아과학회는 뇌수막염 예방접종 지침서를 다음과 같이 업데이트 했다.
  • 2가지 영아용 결합 뇌수막염 백신을 개발한 후 다음과 같이 뇌수막염 예방접종 지침을 개정했다.
  • 건강상 고위험군에 속하는 소아청소년들에게 뇌수막염 예방접종을 해줄 때
  •  생후 2개월부터 4가 MenACWY-CRM (Menveo) 백신이나 2가 HibMenCY-TT (MenHibrix)으로 예장접종을 해 줄 수 있다.
  • 그러나 건강상 고위험군에 속하지 않는 2~10세 아이들에게는 4가 MenACWY-CRM (Menveo) 백신이나 2가 HibMenCY-TT (MenHibrix)으로 기본 예방접종을 해주지 말라고 권장한다.
  • C3, C5, C9 보체 지속적 결핍증이 있거나 기능적 또는 해부학적 무비장을 가진 아이들이나 뇌수막염이 유행하는 지역이나 나라로 또는 뇌수막염이 많이 유행하는 지역으로 여행하는 영아들은 4가 MenACWY-CRM (Menveo) 백신으로 접종받으라고 권장한다.
  • 2~6세 아이들 중 지속적 건강상 위험군에 속하는 아이들이 기본 배열접종을 받은 후 매 5년마다 추가 접종을 받는다.
  • HibMenCY-TT으로 기본 예방접종을 받을 때는 뇌수막염 백신과 HIB 백신으로 예방접종을 동시 접종받는 셈이다.
  • 보통 4회 접종받는다. HIB 백신 접종을 따로 받을 필요가 없다.

  • B형 뇌수막구균 감염병을 예방하는데 쓰는 백신이 새로 개발되었다상품 명은 Trumenba 과 Bexsero이다

    B형 뇌수막구균 감염병을 예방 하기 위해 후기 사춘기 16~18세에 접종하라고 권장한다. 그러나 16~26세에도 접종 할 수 있다

  • Meningococcus infection can lead to meningococcal meningitis ,
  • When meningococcal infections throughout the body follow the bloodstream throughout the body, meningococcal in the blood and meningococcal sepsis occurs.
  • Simultaneous infection of the meninges and the whole body can lead to simultaneous meningococcal meningitis and meningococcal septicemia.
  • According to the meningococcal serum types, meningococcals are classified into group A meningococcus, group B meningococcus, group C meningococcus, group Y meningococcus and group W-135 meningococcus.
  • Meningococcal infectious disease vaccines that can prevent infectious diseases caused by meningococcal A, C, Y, or W-135 serum bacterial infection include MPSV4 vaccine and MCV4 vaccine.
  • In 2015, a vaccine was developed to prevent type B meningococcal infection. MPSV4 vaccine is an abbreviation for Meningococcal polysaccharide vaccine 4 in English.
  • The MENOMUNE-A/C/Y/W-135 vaccine is the brand name of the MPSV4 vaccine. Two-year-old infants and children over two years of age can be vaccinated with the MPSV4 vaccine.
  • The American Academy of Pediatrics recommends that children in high-risk health groups who are highly susceptible to meningococcal infection must receive the vaccination.
  • MCV4 vaccine is an English acronym abbreviation for the Meningococcal conjugate vaccine.
  • The MCV4 vaccine is recommended for children aged 2-10 years old who are at high risk for end-stage complement deficiency, anatomical or functional no-spleen, or high risk of meningococcal infection. You can get the MPSV4 vaccine instead of the MCV4 vaccine.

  • All children under the age of 11 do not receive the meningococcal vaccine by default, but in the United States, in order to prevent infectious diseases caused by meningococcal infection, healthy children and adolescents over the age of 11 are recommended to receive the vaccine by default.

In particular, the meningococcal vaccine should be vaccinated if:

• In the United States, it has been recommended since 2005 to routinely vaccinate healthy adolescent children 11 years and older to prevent infectious diseases caused by meningococcal infection.

• Children over 2 years of age with no spleen or abnormal spleen function

• Children 2 years of age and older with end-stage complement deficiency

• People traveling to areas or countries where meningococcal disease is prevalent

• When a meningococcal infection occurs in the area where you live

• New recruits, college students, especially college students living in dormitories

• People working in hospital clinical laboratories

• etc

Age of meningococcal vaccine vaccination

• MENOMUNE-A/C/Y/W-135 Vaccine is not available to children under 2 years of age.

• It is not recommended that all healthy children under 11 years of age receive routine immunizations with the meningococcal vaccine.

• However, any adolescent or adult aged 11-12 years old can receive the vaccine.

• “Menhibrix Vaccine (June 2012) – A comprehensive vaccine that can prevent sepsis and meningitis caused by Neisseria meningitidis serotypes C and Y and Haemophilus influenzae type b (Hib) by the US FDA.

Additional Information About Meningococcal Vaccine Immunization

• Invasive meningococcal infections affect 2200 to 3000 Americans annually

• 0.9 to 1.5 out of 100,000 infants;

• 1.8 out of 2-4-year-olds;

• Infants 5-17 years old;

• It occurred in 0.7 cases among school-age children and adolescents, and 0.7 cases among young adults aged 18 to 34 years.

• The types of meningococci are subdivided into 13 types according to the serum species.

• Most invasive meningococcal infections result from meningococcal infections belonging to the A, B, C, Y, and W-135 serobacteria species.

• There is a vaccine that can prevent infectious diseases caused by meningococcal infection belonging to A, C, Y, and W-135 serobacteria.

• In the United States, it is not yet routinely recommended to vaccinate healthy children under the age of 11 to prevent diseases caused by meningococcal infection. However, a new vaccine has just been released.

• Healthy adolescent children 11-12 years of age and older and military personnel are routinely recommended to be vaccinated. Adolescent children aged 11-12 years and older who are more susceptible to this meningococcal infection;

• College students living in school dormitories;

• Parents are encouraged to get their children vaccinated against meningococcal infection, or who are traveling to countries where the disease is endemic.

• Most mothers and fathers of children who have recovered or died from severe invasive meningococcal infection “was unaware that there was a vaccine available to prevent this disease.” There is also research.

• I want all mothers and fathers to know that there is a vaccine for this type of immunization and that it can prevent invasive meningococcal disease. Vaccination against meningococcal infection 10/2015

• The American Academy of Pediatrics has updated the Meningitis Vaccination Guidelines as follows.

• After developing two infant combined meningitis vaccines, the guidelines for meningitis vaccination were revised as follows.

• When immunizing children and adolescents who are at high health risk for meningitis

• From 2 months of age, enteric vaccination can be given with tetravalent MenACWY-CRM (Menveo) vaccine or bivalent HibMenCY-TT (MenHibrix).

• However, it is recommended that children aged 2 to 10 years who are not at high risk for health should not receive primary immunization with the quadrivalent MenACWY-CRM (Menveo) vaccine or the bivalent HibMenCY-TT (MenHibrix) vaccine.

• Children with a persistent C3, C5, or C9 complement deficiency, functional or anatomical splenomegaly, or infants traveling to areas or countries where meningitis is endemic It is recommended to be vaccinated with

• Children 2-6 years of age who are at persistent health risk receive a booster dose every 5 years after receiving the primary sequence.

• When you receive the basic vaccination with HibMenCY-TT, you are vaccinated with the meningitis vaccine and the HIB vaccine at the same time.

• A new vaccine has been developed to prevent meningococcal B type. The trade names are Trumenba and Bexsero. In order to prevent meningococcal type B infection, it is recommended to be vaccinated at the age of 16 to 18 in late puberty. However, it can be vaccinated at the age of 16-26.
